Liability Waiver
All class participants are required to sign a liability waiver for themselves and for any minors attending with them. Please download the forms below, sign and date where indicated, and bring these with you.
If you are able to scan your signed original and email it to us before the event, we will keep it on file. If you arrive without a signed waiver you will be required to sign one on-site before you participate. No exceptions.
